OFDA Funeral Home Membership Application

The funeral home(s) listed on the application is applying for membership in the Ohio Funeral Directors Association (“OFDA”).
Eligibility: A funeral home that is duly licensed under the laws of the state of Ohio may apply for membership in OFDA as a Firm Member. An applicant must receive an affirmative vote from the OFDA Executive Director and Secretary/Treasurer in order to be admitted into membership.
Application: Please complete the application below in its entirety. If the firm member applicant also has one or more branch funeral homes under common ownership, the application should list each of the branch member applicants. Following OFDA staff processing, the application will be reviewed by the OFDA Executive Director and Secretary/Treasurer.
Dues Schedule: Dues are calculated by a base fee, plus the number of death certificates filed for all locations of the firm during the previous calendar year. The price per death certificate is categorized in tiers below. Death certificate amounts go through each applicable tier at that rate.
EXAMPLE: 650 death certificates:
Base Fee (one base fee per company, not per branch location) = $220
500 death certificates at Tier 1 (x $3.50) = $1,750
150 death certificates at Tier 2 (x $1.75) = $262.50
Total Due = $2,232.50
Upon approval of your membership application you will receive an invoice to pay your dues based on a prorated amount depending on the time of year your application is approved.
Annually, OFDA members are mailed dues invoices in November. Deadline for remittance is February 28th.
Terms and Conditions: As a licensed Ohio funeral director representing the funeral home(s) applying for membership within the OFDA, the applicant agrees as a condition of membership that the applicant will abide by the OFDA Bylaws that have or may be adopted by OFDA.
